Clearance Checklist

1. Pennsylvania Access to Criminal History clearance (no fee for volunteers).

  • If you have one already, see Step 5. Criminal history reports shall be no more than five (5) years old at the time of employment

  • You can complete a new clearance request online at https://epatch.pa.gov/home

  • Choose “New Volunteer Record Check.”

  • In the field “Volunteer Organization Telephone Number,” please enter 310-505-5355.

2. Child Abuse Clearance issued by the Department of Public Welfare ($8 online).

  • If you have one already, see Step 5. Chester Upland Youth Soccer requires that the report be no older than three (3) years old at the time of employment

  • You can complete it online at https://www.compass.state.pa.us/cwis/public/home

  • Create individual account.

  • Reimbursement available upon request 

3. Federal Bureau of Investigations (FBI) Criminal History Report  ($25.75)

  • If a volunteer has resided in PA continuously for the last 10 years, then s/he will NOT need to obtain an FBI clearance

  • Clearances remain valid for three (3) years for paid staff and five (5) years for volunteers

  • To schedule your FBI fingerprints appointment go to https://uenroll.identogo.com/ and enter the following service code: 1KG756

  • You’ll also need this information: 

    1. Employer= Chester Upland Youth Soccer

    2. Address= 311 Baltimore Ave, Media PA 19063

  • You can complete it online here

  • Reimbursement available upon request 

4. Create a US Soccer Foundation Resources Hub profile at http://ussoccerfoundation.org/resourcehub

  • Program ID = Che0059

  • Complete the Background Check using the large red button.

  • Complete the Standard Soccer for Success Training.  (Please follow the Nutrition-Education tract that can be found in “Quick Links.” 

  • Complete the Safe Sport Abuse Awareness Training.

  • Download a copy of the Safe Sport Abuse Awareness Training so that you can upload it for us. (See Step 5.)

  • Download the Coach-Mentor app as instructed in the training.

5. Upload all clearance documentation at https://forms.gle/VQpmUgXg4LF8ZA436

6. Once the above is completed, we will be in touch about getting you on payroll if you have selected a paid position.
